What you will do

  • Deliver intensive, wrap-around, trauma-informed support to people experiencing homelessness and multiple disadvantage.
  • Maintain accurate case-management records covering support plans, safety plans, progress, and outcomes.
  • Review person-centred support plans and monitor progress across an assigned caseload.
  • Liaise with adult social care, criminal justice, health, employment, and training agencies to coordinate support.
  • Build professional networks to promote the Housing First pilot and advocate for its clients.
  • Participate in evening, weekend, and bank holiday rota duties and provide temporary colleague cover.

Requirements

  • Significant experience supporting people affected by homelessness, substance misuse, mental or physical ill health, and offending behaviour.
  • Experience liaising effectively with social, health, housing, and criminal justice agencies.
  • Experience writing client records, completing monitoring materials, and contributing to project evaluation.
  • Understanding of risk and needs assessment, support planning, goal setting, review processes, and service quality.
  • Ability to engage people who have low interaction with or suspicion of services, using empathy and a trauma-informed approach.
  • Strong relationship-building, advocacy, problem-solving, flexibility, and resilience when working with challenging situations.
