Social Worker - MSW, LICSW Hospice Based

Location: Community-based hybrid role serving Lawrence, Lowell, Malden, and New Hampshire locations. Full-time hours are 8:00 a.m.–4:30 p.m.

Salary: $66,397.24–$82,991.27 per year

What you will do

• Provide psychosocial assessments, treatment plans, therapy, counseling, and psychoeducation for patients and families.
• Assess and intervene in crisis situations involving suicide, homicide, family violence, and elder or child abuse.
• Evaluate long-term care and home-care needs, develop care plans, and coordinate placements when necessary.
• Connect patients and families with hospital, community, governmental, legal, and social-service resources.
• Collaborate with healthcare professionals on coordinated care, discharge planning, informed decision-making, and continuity of care.
• Maintain clinical documentation, participate in quality improvement and research, and contribute to education, policy development, and professional committees.

Requirements

• Master’s degree in Social Work from an accredited institution.
• Licensed Clinical Social Worker (LCSW); New Hampshire licensure may be required depending on the assigned territory and operational needs.
• At least one year of related experience.
• Knowledge of social work theory, casework, treatment and case-management methods, healthcare systems, and community resources.
• Strong diagnostic, counseling, analytical, organizational, interpersonal, communication, and computer skills.
• Ability to read, write, and communicate in English.
