Social Worker MSW- Geropsyche- Full Time- Days

Location: Manchester, New Hampshire, United States. Monday–Friday, 8:30 AM–5:00 PM, with patient care and occasional off-site coordination.

What you will do

• Conduct clinical social work assessments and coordinate care with nurses, physicians, case managers, and multidisciplinary teams.
• Provide supportive counseling, crisis intervention, emotional support, patient advocacy, and education for patients and families.
• Monitor clinical progress, participate in daily rounds, and support integrated care pathways and quality improvement activities.
• Coordinate utilization management, address delays in care, and help optimize length of stay and resource use.
• Develop comprehensive discharge plans for socially and medically complex patients, including community services, hospice, nursing home placement, and in-home care.
• Facilitate family meetings, advance care directive counseling, support groups, and coordination with community social service agencies.

Requirements

• Master of Science in Social Work or a closely related field.
• One to three years of experience in a healthcare system, preferably a hospital.
• Ability to work with patients experiencing acute and chronic psychiatric disorders, including under stress and frequent interruptions.
• Ability to collaborate with healthcare and community teams and exercise professional judgment in complex situations.
• Excellent clinical documentation and experience using EMR systems are preferred.
• Approved aggression de-escalation training is required; NH LICSW is preferred.
