Focus on coordinating community resources, supporting care transitions, addressing caregiver and practical needs, and advocating for patient autonomy, dignity, and comfort.

Location: Southern New Hampshire territories, including Merrimack, Pelham, Salem, and Nashua; patient visits take place in homes, hospitals, skilled nursing facilities, assisted living facilities, and the High Point House hospice inpatient unit in Haverhill, Massachusetts.

Pay range: $72,371.94–$90,464.92 per year.

What you will do

  • Provide specialized hospice social work services to patients and families facing serious illness, end-of-life care, and bereavement.
  • Conduct psychosocial assessments and provide counseling, coping and adjustment support, and grief services.
  • Support goals-of-care discussions and advance care planning while advocating for patient autonomy, dignity, and comfort.
  • Identify psychosocial, emotional, caregiver, financial, cultural, and practical needs related to terminal illness and decline.
  • Coordinate community resources, care transitions, and discharge planning with patients, caregivers, and the interdisciplinary hospice team.
  • Visit patients across homes, hospitals, skilled nursing facilities, assisted living facilities, and the hospice inpatient unit.

Requirements

  • Master’s degree in Social Work from an accredited institution.
  • Licensed Clinical Social Worker (LCSW); New Hampshire licensure may be required depending on the assigned territory and operational need.
  • At least one year of related experience.
  • Knowledge of social work theory, treatment and case management methods, healthcare and social policies, and community resources.
  • Strong diagnostic, counseling, interpersonal, analytical, organizational, and communication skills.
  • Ability to read, write, and communicate in English; full-time availability from 8:00 a.m. to 4:30 p.m.
