hirify.global’s Care Coordination Department at Elliot Hospital serves patients and families through clinical social work and coordinated healthcare services.

What you will do

• Provide clinical social work interventions for neonates, children, adolescents, adults, older adults, and their families.
• Assess patient status and coordinate care with nurses and multidisciplinary teams.
• Help develop, implement, and evaluate integrated clinical pathways.
• Monitor patient progress to ensure timely and appropriate care.
• Develop discharge plans and coordinate referrals with interdisciplinary teams.
• Conduct utilization reviews focused on quality outcomes and cost-effective care.

Requirements

• Master of Science in Social Work required.
• New Hampshire LICSW preferred.
• 1–3 years of experience in a healthcare system, preferably a hospital.
• Proficiency in assessing treatment responses and adapting care for diverse patient populations.
• Strong clinical documentation skills and experience using electronic medical record systems preferred.
• On-site work in Manchester, New Hampshire is required.
