Social Worker LICSW - Pediatric Primary Care

Location: Tufts Medical Center; work includes office, inpatient and outpatient clinic, home, and community settings.

Pay range: $60,908.12–$76,140.43 annually.

What you will do

• Provide psychosocial and psychiatric social work assessments for patients and families, including crisis assessments involving abuse, violence, suicide, or homicide risks.
• Develop treatment plans and deliver therapy, counseling, psycho-education, and short-term medical social services using individual, family, and group modalities.
• Assess long-term care, home-care, discharge, and placement needs while coordinating continuity of outpatient and community care.
• Connect patients and families with hospital, governmental, legal, educational, and community resources.
• Collaborate with multidisciplinary healthcare teams and external agencies to integrate psychosocial factors into treatment and recovery plans.
• Maintain clinical documentation, participate in quality improvement and program development, and support professional education and supervision.

Requirements

• Master’s degree in Social Work.
• Licensed Independent Clinical Social Worker (LICSW) credential.
• Knowledge of social work theory, casework, treatment methods, health and mental-health resources, and community services.
• Diagnostic, counseling, analytical, interpersonal, organizational, and communication skills.
• Ability to communicate in English and use general office, spreadsheet, database, and presentation software.
• Ability to work with patients across the lifespan and engage patients and caregivers in acute treatment and discharge planning.
