RN Case Manager I - Telephonic / Field - Northeast Kansas

Location: Must reside in Kansas; field-based work supporting Wyandotte, Johnson, and Miami counties in Northeast Kansas, with 10–15% travel to visit members in person and occasional office attendance.

What you will do

• Perform care management for Kansas Medicaid members with complex and chronic care needs.
• Conduct virtual and in-person assessments at hospitals, community centers, homes, and other designated locations.
• Develop, implement, coordinate, monitor, and evaluate individualized care plans.
• Facilitate authorizations and referrals and coordinate internal and external resources.
• Collaborate with Medical Directors and Physician Advisors on treatment plans.
• Help resolve provider, claims, reimbursement, and service issues.

Requirements

• BA/BS in a health-related field, plus at least 3 years of clinical experience, or an equivalent combination of education and experience.
• Current, unrestricted RN license in Kansas required.
• Residence in Kansas required.
• Computer proficiency is strongly preferred.
