Location: Southern New Hampshire area, including Nashua, Hudson, Pelham, and Salem; services are provided across inpatient, ambulatory, and community patient-care settings.
Salary: $77,333.26–$98,603.60 annually, based on full-time equivalent.

What you will do
- Provide psychosocial assessments and develop treatment plans for patients and families.
- Deliver individual, family, and group therapy, counseling, psychoeducation, and crisis interventions.
- Assess needs related to long-term care, home care, hospitalization, illness, finances, and community support.
- Coordinate discharge planning and outpatient psychosocial care to support continuity of treatment.
- Collaborate with healthcare teams, agencies, courts, schools, clinics, and community resources.
- Advocate for patients and families, maintain clinical documentation, support quality improvement, and contribute to education and program development.
Requirements
- Master’s degree in Social Work from an accredited institution.
- Licensed Independent Clinical Social Worker (LICSW); New Hampshire licensure may be required depending on the assigned territory and operational need.
- Five years of related experience.
- Knowledge of social work theory, casework, treatment methods, healthcare systems, mental health, disability services, and community resources.
- Strong diagnostic, counseling, interpersonal, organizational, analytical, and communication skills.
- Ability to read, write, and communicate in English.
Nice to have
- Experience in inpatient psychiatric or mental health settings.
- Experience working with major mental illness, dementia, or older adults.
