Licensed Practical Nurse - Pepperell Family Practice - Full Time - Foundation Medical Partners

Targeted pay range: $24.81–$39.70 per hour

What you will do

• Provide professional nursing care under the supervision of registered nurses, providers, and the practice manager.
• Perform nursing assessments, interventions, and progress evaluations during in-person, telephone triage, and telehealth encounters.
• Help design and implement patient care plans with an interdisciplinary team of nurses, physicians, advanced-practice nurses, physician assistants, and other health professionals.
• Educate patients on self-management, medications, nutrition, health, and wellness.
• Administer prescribed medications, treatments, and vaccines; perform point-of-care testing; and collect and handle specimens within the scope of practice.
• Document nursing care accurately and in detail in the medical record.

Requirements

• Current LPN license in New Hampshire or a compact state.
• Current BLS/CPR certification.
• Graduate from an accredited nursing school, or currently enrolled in an accredited registered nursing program while holding a current New Hampshire LPN license.
• Full-time, onsite position in Pepperell, Massachusetts, working Monday through Friday from 7:30 a.m. to 4:30 p.m.
