LPN Outpatient - Tewksbury Family Health

Location: Tewksbury Family Health, 1574 Main St. Suite 200, Tewksbury, Massachusetts 01876. Full-time schedule: 40 hours per week, Monday–Friday, 8:30 a.m.–5:00 p.m.

Salary: $30.00–$33.59 per hour.

What you will do

• Provide outpatient nursing care, interventions, medication administration, and treatments under the direction of a registered nurse.
• Deliver direct patient care within the primary nursing model while maintaining patient rights, dignity, and continuity of care.
• Assess and manage pain, monitor patient responses, and document nursing interventions in the medical record.
• Communicate changes in patient condition and pertinent observations to registered nurses, physicians, clinical managers, and charge nurses.
• Educate patients and families about medications, treatments, activities of daily living, and discharge instructions.
• Participate in delegation, performance improvement activities, professional learning, and collaborative healthcare-team work.

Requirements

• Massachusetts Licensed Practical Nurse (LPN) license required.
• Basic Life Support certification required.
• Experience is determined by unit requirements and operational needs.
• Knowledge of the nursing process, patient assessment, pharmacology, medication administration, and age-appropriate care.
• Ability to communicate effectively with patients, families, physicians, and the healthcare team.
• Ability to stand and walk for extended periods, transfer patients and equipment, and handle exposure to clinical hazards and emergency situations.
