Home Health - LPN

Location: Lowell, Massachusetts and surrounding areas; in-home patient care required

Salary: $31.00–$36.61 per hour

What you will do

• Provide professional and nonprofessional nursing care to patients and families in their homes under physician orders and RN Case Manager supervision.
• Assess physical and psychosocial needs, implement evidence-based nursing interventions, and monitor patient responses.
• Follow and communicate updates to individualized nursing care plans in collaboration with RN case managers, physicians, rehabilitation staff, social services, home health aides, and other care providers.
• Document assessments, interventions, communications, verbal orders, and patient data in computerized and Medicare-compliant records.
• Provide patient and family education, referrals, advocacy, and support for independence and discharge planning.
• Maintain clinical competence, follow safety and confidentiality procedures, and assist with orientation and field demonstrations when assigned.

Requirements

• High school diploma or equivalent and graduation from an accredited practical nursing program.
• Current Licensed Practical Nurse (LPN) license required.
• At least one year of acute nursing experience.
• Basic Life Support certification.
• State-issued driver's license and personal auto insurance.
• Ability to work full time for 40 hours per week, including weekend and holiday commitments; IV certification may be required depending on role needs.
