Salary: $124,800–$156,000 per year

What you will do

• Develop meaningful relationships with empaneled patients.
• Meet patients’ primary care needs in an internal medicine practice.
• Provide proactive care management, preventive medicine, and integrated population health services.
• Collaborate with physicians, specialists, and multidisciplinary members of the medical community.
• Deliver connected academic and community-based care to patients in the local community.

Requirements

• Graduate of an accredited or approved Nurse Practitioner program.
• At least 2 years of Nurse Practitioner experience in primary care or a similar clinical setting.
• Current Registered Nurse and Nurse Practitioner licenses in the Commonwealth of Massachusetts.
• Excellent verbal and written communication skills.
• Patient-centered approach with a focus on high-quality care and patient satisfaction.
• Passion for primary care and ability to work collaboratively across disciplines.
