TL;DR
Nurse Practitioner (Healthcare): Conducting in-home wellness and risk adjustment assessments for diverse patient populations across Oregon with an accent on comprehensive examinations, preventive screenings, medication reconciliation, and clinical documentation. Focus on identifying diagnoses and care gaps, educating patients about chronic conditions, and coordinating findings with primary care teams while traveling within an assigned service area.

Location: Coeur d'Alene, Idaho, United States. Workplace: On-site. Full-time providers support the entire state of Oregon, with daily travel within a 55-mile radius of the assigned home location and scheduled statewide travel with at least two weeks' notice.

What you will do

  • Travel to members' homes and conduct comprehensive health evaluations, Annual Wellness Visits, and chronic-condition assessments.
  • Perform medical histories, physical examinations, review of systems, vital-sign collection, medication reconciliation, and preventive screenings.
  • Provide education and counseling on preventive care, health monitoring, medication adherence, and chronic-condition management.
  • Identify clinically relevant diagnoses, document findings in the EMR, and prepare information for primary care providers.
  • Help close quality care gaps through screenings, laboratory work, follow-ups, and care coordination.
  • Protect patient information under HIPAA and maintain accurate, timely, compliant documentation.

Requirements

  • Active, unencumbered Nurse Practitioner license in Oregon.
  • At least 3 years of direct patient-care experience; primary care or adult/geriatric experience is preferred.
  • Ability to perform in-home clinical assessments and travel throughout Oregon.
  • EMR proficiency is desired.
  • Bilingual proficiency is an advantage.
