Nurse Practitioner - Primary Care

Location: Delafield, Wisconsin, United States; in-person and virtual patient visits with on-site credentialing required.

What you will do

• Provide comprehensive primary care through in-person and virtual visits.
• Diagnose conditions, prescribe medications, and create personalized health plans.
• Educate patients on wellness, prevention, and lifestyle management.
• Manage early intervention, chronic conditions, and long-term wellness.
• Collaborate with physicians, medical assistants, and other clinicians.
• Contribute to a positive, team-oriented clinic culture.

Requirements

• Active, unrestricted Family Nurse Practitioner license in the practicing state.
• Board certification as a Family Nurse Practitioner.
• DEA registration.
• Familiarity with HIPAA and commitment to patient confidentiality.
• Ability to complete on-site credentialing.
• Two or more years of primary care experience preferred.
