TL;DR
State Traveler Provider (Nurse Practitioner) (Healthcare): Conducting in-home wellness visits, Annual Wellness Visits, and risk adjustment assessments for Medicare and other eligible populations with an accent on clinical documentation, preventive screenings, and patient education. Focus on traveling throughout Southern Illinois, identifying care gaps, coordinating follow-up care, and maintaining accurate HIPAA-compliant EMR records.

Location: Belleville, Illinois, United States; on-site. The role requires approximately 75% travel to members throughout Southern Illinois, with possible overnight stays.

Salary: $130,000 annually, plus a sign-on bonus.

What you will do

  • Conduct in-home health assessments, Annual Wellness Visits, and preventive screenings for Medicare and other eligible members.
  • Review medical histories, perform physical examinations, collect vital signs, and complete medication reconciliation.
  • Provide education and counseling on disease prevention, medication adherence, chronic condition management, and care goals.
  • Identify diagnoses and care gaps supporting risk adjustment, care management, and quality improvement.
  • Document findings accurately in the EMR and communicate information for post-visit summaries.
  • Collaborate with interdisciplinary teams to improve clinical quality measures and patient outcomes.

Requirements

  • Active and unrestricted Illinois Nurse Practitioner license.
  • National board certification applicable to the NP specialty.
  • At least 3 years of direct patient care experience in primary care, family practice, internal medicine, geriatrics, urgent care, or a related setting.
  • Valid driver's license, reliable transportation, and willingness to travel extensively throughout Southern Illinois.
  • Ability to work in patient homes and to lift and carry clinical equipment as needed.
  • Ability to maintain compliant documentation, confidentiality of protected health information, and current professional licensure and certifications.

Nice to have

  • Experience with Medicare populations or in-home assessments and risk adjustment visits.
  • EMR proficiency.
  • Bilingual language skills.
  • Familiarity with value-based care and quality gap closure initiatives.
