Location: Onsite at Tufts Medical Center Community Care, 50 Rowe Street, Suite 600, Melrose, Massachusetts 02176

Salary: $124,800–$156,000 per year

What you will do

• Evaluate, diagnose, and manage a broad range of cardiovascular conditions in an outpatient setting.
• Develop longitudinal relationships with patients and support their ongoing care.
• Collaborate with cardiologists, primary care physicians, nurses, and other healthcare professionals.
• Optimize treatment plans for acute and chronic cardiac disease.
• Advance quality outcomes, preventive care, chronic disease management, and patient quality of life.
• Work with community stakeholders and multidisciplinary teams to provide coordinated, patient-centered care.

Requirements

• Graduate of an accredited or approved Nurse Practitioner or Physician Assistant program.
• Current Nurse Practitioner or Physician Assistant license in the Commonwealth of Massachusetts.
• Current CPR certification.
• Cardiology, primary care, internal medicine, or related advanced practice experience is preferred.
• Ability to work cooperatively with a broad range of stakeholders and disciplines.
