What you will do

• Provide virtual primary care visits, including comprehensive assessments, chronic disease management, and acute care interventions.
• Diagnose and manage chronic conditions such as hypertension, diabetes, and COPD.
• Order and interpret diagnostic tests, imaging, and coordinate follow-up care.
• Manage medication prescriptions, titrations, and refills while providing health coaching.
• Collaborate with multidisciplinary teams to facilitate specialist referrals and home-based services.
• Provide feedback on the electronic health platform to enhance the provider and patient experience.

Requirements

• Active Nurse Practitioner (APRN) license in Washington and the ability to obtain licensure in multiple states within 90 days.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary care, urgent care, or internal medicine.
• Active BLS certification at time of hire.
• Ability to work Pacific Time (PT) hours (Monday-Friday, 8:30 am – 5:00 pm).

Nice to have

• Experience with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings).
• Proficiency with EHRs and virtual care workflows.
• Previous experience in telehealth or virtual care settings.
