Visiting Nurses Association - Licensed Practical Nurse - Home Care - Full Time

Location: Manchester, New Hampshire, United States; travel throughout southern New Hampshire is required for home health care visits.

Bonus: $7,500 eligible.

What you will do

• Deliver home health care to patients of all ages with basic to complex and changing needs.
• Assess patients’ physical and psychosocial status, medical information, and home environments.
• Participate in developing patient care plans under the direction of a Home Care RN.
• Document and communicate patient status and care needs to nurses, physicians, managers, patients, families, and partner agencies.
• Provide dignified, supportive, and therapeutic nursing care in the home.
• Obtain prior authorizations, explain insurance coverage and costs, and maintain documentation to prevent write-offs.

Requirements

• Graduate of an accredited Licensed Practical Nursing program.
• At least 2 years of previous clinical experience; home care experience is preferred.
• Current New Hampshire LPN licensure required.
• Current BLS certification required, or certification must be obtained within 90 days of hire.
• Valid driver’s license and automobile insurance required for travel throughout southern New Hampshire.
• Availability for 8–12-hour shifts, including weekend, evening, and night coverage as needed; every-other-weekend rotation applies.
