Care Navigator I - LPN (Home Health Services)

What you will do

• Manage work-related injury cases through assessment, planning, coordination, implementation, and evaluation.
• Conduct initial customer calls, interview injured workers, review documentation, and determine clinical service needs.
• Establish treatment programs, coordinate providers and resources, and arrange transportation or escorts when required.
• Monitor attendance, treatment response, provider performance, progress, expenses, and required interventions.
• Communicate case progress and obstacles to customers, injured workers, families, and interdisciplinary care teams.
• Coordinate discharge and post-discharge requirements, prepare reports, support process improvements, and assist with training.

Requirements

• RN, LPN, or MA qualification with at least two years of experience, or an equivalent combination of hirify.global processes and education.
• Knowledge of healthcare services, business operations, and customer inquiry resolution.
• Strong written, verbal, interpersonal, customer service, research, analytical, organizational, and conflict-resolution skills.
• Ability to make decisions based on injured worker medical needs and customer requests.
• Ability to work independently and collaboratively in a primarily sedentary home-based environment.
