Home Health Registered Nurse, RN Case Manager

Location: Lowell, Massachusetts and surrounding areas; patient care is provided in clients’ homes and the community.

Salary: $84,300.22–$107,481.20 per year.

What you will do

• Assess patients’ physical and psychosocial needs and develop individualized plans of treatment.
• Deliver comprehensive home health nursing, symptom management, supportive care, and clinical interventions.
• Coordinate care, case conferences, referrals, and discharge planning with patients, families, physicians, community resources, and interdisciplinary teams.
• Manage an assigned caseload, prioritize visits and calls, and provide telephone triage and caregiver support to help prevent hospitalization.
• Complete physician orders, OASIS documentation, medication reconciliation, and timely patient records.
• Provide rotating evening, night, weekend, and holiday coverage while following home and community safety procedures.

Requirements

• Massachusetts RN and New Hampshire RN licensure required.
• Valid state-issued driver’s license required for field-based home care.
• At least one year of acute medical/surgical nursing experience.
• Basic Life Support certification.
• Knowledge of home health and hospice standards, symptom management, pharmacology, medication administration, and care planning.
• Ability to communicate, read, and write in English required. Must work autonomously, manage time effectively, and prioritize a busy caseload.

Nice to have

• Bachelor of Science in Nursing and specialty nursing certifications.
• Three years of acute nursing experience, including medical/surgical, oncology, hospice, or home health.
• Experience with IV therapy and complex wound care.
