Care Manager, RN (New Haven, CT / Field-based)

Location: Field-based in New Haven, Connecticut, with frequent travel to patients' homes within the assigned geographic area.

Salary: $90,000–$100,000 per year

What you will do

• Conduct initial in-person clinical assessments and coordinate service activation.
• Develop, implement, manage, and revise individualized patient care plans.
• Coordinate care with physicians, interdisciplinary teams, community resources, patients, and caregivers.
• Provide caregiver coaching, education, and connections to appropriate resources.
• Review caregiver notes, monitor changes in patient condition or environment, and initiate appropriate follow-up.
• Maintain compliance with regulatory standards, company policies, and care procedures.

Requirements

• Diploma from an accredited nursing program; new graduates are welcome.
• Valid and active Registered Nurse license in the State of Connecticut.
• Ability to travel by car within the assigned geographic area; a valid driver’s license and car insurance are required.
• Ability to prepare effective case summaries and communicate clinical information clearly.
• Proficiency with case management systems, Microsoft Office, and other web-based applications, including the ability to teach others.
• Strong customer service skills, empathy, active listening, and patience.
