Care Manager (LMSW)

Location: Remote in Michigan with up to 75% local travel to support members in Wayne and Macomb counties. The remaining time may be worked remotely from home. Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.

What you will do

• Assess complex member needs through phone, digital, in-home, and other site visits.
• Develop and maintain personalized care plans for members with high-acuity mental health and substance use disorder needs.
• Coordinate access to providers, specialists, caregivers, community resources, and healthcare services.
• Monitor treatment outcomes, clinical symptoms, complications, and side effects, updating care plans as needed.
• Educate members and families about treatment options, referrals, provider instructions, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve quality and cost-effective care delivery.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional, such as LMSW, LCSW, LMFT, LMHC, or LPC, or an RN license according to state contract requirements.
• Behavioral health experience is required.
• Ability to complete up to 75% local travel in Wayne and Macomb counties.
