Care Coordinator - Portales /Clovis, NM

Location: Remote work in the community areas of Clovis and Portales, New Mexico; candidates must reside near these areas.

Salary: $50,225–$75,335 per year, with potential eligibility for short-term incentives and benefits.

What you will do

• Coordinate care for members with behavioral health conditions who require intensive interventions and oversight.
• Conduct comprehensive health risk and needs assessments covering psychosocial, physical, medical, behavioral, environmental, and financial factors.
• Develop, communicate, document, and monitor individualized care plans and backup plans.
• Coordinate medical, behavioral, social, community-based, home care, and long-term care resources.
• Identify gaps in care, measure intervention effectiveness, analyze trends, and generate care coordination reports.
• Collaborate with members, families, physicians, clinicians, providers, legal representatives, and interdisciplinary care teams.

Requirements

• 3–5 years of experience in social work, nursing, healthcare, or relevant equivalent experience.
• Experience in utilization management, quality assurance, home or facility care, community health, long-term care, or occupational health.
• Knowledge of referral coordination with community and private or public resources.
• Ability to analyze data and trends, perform cost-benefit analysis, negotiate, and support cost containment.
• Ability to make decisions requiring significant analysis and original thinking in complex situations.
• High school diploma or GED and a valid in-state driver’s license required.

Nice to have

• Associate or bachelor’s degree.
• Certified Case Manager certification.
• Licensed Clinical Social Worker license.
• Registered Nurse state or compact-state licensure.
