Field Care Manager, Behavioral Health 2

Location: Remote South Carolina with mandatory field work and home or facility visits; travel up to 50% of the time.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members' behavioral, physical, environmental, and psychosocial health needs.
• Guide members and families to appropriate care and community resources.
• Develop member care plans and manage care toward desired outcomes.
• Use behavioral change, health promotion, coaching, and wellness strategies to address care barriers.
• Visit member homes or facilities and collaborate with internal partners and care resources.

Requirements

• Active, unrestricted South Carolina LMSW license or active, unrestricted compact RN license.
• At least 2 years of experience as a behavioral health professional.
• Must reside and perform work in South Carolina and be willing to travel.
• Valid state driver's license and personal vehicle liability insurance are required.
• Experience with behavioral change, health promotion, coaching, and wellness.
• Proficiency with electronic information systems and Microsoft Office tools, including Word, Outlook, and Excel.

Nice to have

• Experience supporting patients by telephone.
• Experience in community mental health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.
• Location in the Upstate or Midlands area.
