Field Care Management Behavioral Health - Field/Remote LA (Behavioral Health)

Location: Remote Louisiana with onsite/home member visits; occasional travel to company offices may be required for training or meetings. Monday through Friday, 8:00 AM to 5:00 PM, 40 hours per week.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and face-to-face behavioral, physical, environmental, and psychosocial assessments.
• Create individualized member care plans and monitor progress toward desired outcomes.
• Identify and resolve barriers affecting effective care and member wellness.
• Coordinate behavioral health, physical health, social determinant of health, and value-added services.
• Collaborate with primary care providers, community services, and the transdisciplinary care team on transitions of care.
• Submit incident reports and contribute to continuous improvement of the member experience.

Requirements

• 3+ years of experience as an LPC, LCSW, LAC, or psychiatric RN.
• Applicable state licensure in the relevant field of study.
• Intermediate to advanced computer skills and experience with Microsoft Office.
• Ability to work onsite during home member visits and maintain a dedicated workspace protecting member PHI and HIPAA information.
• Ability to work a 40-hour Monday–Friday schedule from 8:00 AM to 5:00 PM.
• Home internet with at least 25 Mbps download and 10 Mbps upload speed.
