Field Care Manager, Nurse

Location: Remote from Illinois or a border state within a commutable distance of Illinois; requires up to 25% field travel and occasional onsite meetings at hirify.global locations in Illinois. Work hours are Monday-Friday, 8:00 AM-5:00 PM CST, with some schedule flexibility based on business needs.

Salary: $71,100-$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Coordinate care for members with acute, chronic, complex, behavioral health, and medically fragile conditions.
• Conduct telephonic and face-to-face assessments and health risk evaluations across the lifespan.
• Create individualized care plans and coordinate medical, behavioral, functional, and psychosocial services.
• Identify and resolve barriers to care while monitoring progress toward desired outcomes.
• Collaborate with members, caregivers, providers, medical directors, interdisciplinary teams, and community resources.
• Maintain contractual and regulatory compliance while making independent decisions within established guidelines.

Requirements

• Must reside in Illinois or a border state within a commutable distance of Illinois.
• Associate degree in nursing and at least 3 years of nursing experience, including 2 years of care management experience.
• At least 1 year of acute care clinical experience in an approved setting such as a hospital, emergency department, home health, rehabilitation, ambulatory care, behavioral health, case management, disease management, utilization management, or clinical triage.
• Knowledge of community health, social service agencies, and community resources.
• Ability to use electronic medical records and other electronic information systems, with strong keyboard and web navigation skills.
• Valid state driver's license, personal vehicle liability insurance, and willingness to travel up to 25%.

Nice to have

• Bachelor of Science in Nursing and 3+ years of in-home assessment and care coordination experience.
• Managed care, health promotion, coaching, wellness, and chronic or complex condition management experience.
• Case management, Motivational Interviewing, CCM, ACM, or related certification.
• Bilingual ability in Spanish, Arabic, Vietnamese, or another language.
• Experience working with diverse populations across the lifespan.
