Behavioral Health Case Manager II (Healthcare)

Location: Must reside in Connecticut, United States. Hybrid schedule with 1–2 days per week in the office; candidates must live within a reasonable commuting distance of the posting location. Work shift: Monday through Friday, 8:30 a.m.–5:00 p.m. EST.

What you will do

• Provide telephonic behavioral health case management within the scope of licensure.
• Navigate behavioral health, substance use, and community-based resources for members.
• Coordinate referrals, appointments, follow-up outreach, and connections to recommended services.
• Assess member needs using screening criteria and clinical judgment, then develop individualized care plans.
• Coordinate with providers, patients, community organizations, and internal resources to support access to quality and cost-effective care.
• Serve as a resource to other behavioral health case managers.

Requirements

• MA/MS in social work, counseling, or a related behavioral health field, or a nursing degree, with at least 3 years of clinical experience in social work counseling involving complex psychiatric and substance use disorder treatment.
• Current, active, unrestricted license to practice as an RN, LCSW, LMHC, LICSW, LPC, LMFT, or Clinical Psychologist in Connecticut.
• Experience in case management and telephonic or in-person coaching for members with complex psychiatric, substance use, or medical disorders.
• Managed care experience.
• Strong communication, motivational interviewing, customer service, clinical assessment, and care coordination skills.

Nice to have

• Dual licensure in mental health and substance use treatment.
• Knowledge of Connecticut behavioral health, substance use, and community resources.
• Experience with telephonic care coordination, resource navigation, or referral services.
• Ability to select referrals based on clinical needs, insurance coverage, geography, provider availability, and member preferences.
