Care Manager (BH - Foster Care)

Location: Remote from anywhere within the state of Missouri. Monday–Friday, 8:00 a.m.–5:00 p.m.; phone, digital, home, and other site visits may be required.

Salary: $56,200–$101,000 per year

What you will do

• Develop, assess, and facilitate complex care management activities for behavioral health needs.
• Create personalized care plans and provide education to members, families, and caregivers.
• Assess member needs through telephonic, digital, in-home, and other site-based outreach.
• Identify and coordinate behavioral health providers, specialists, community resources, and services.
• Monitor care plans, treatment effects, clinical symptoms, and member outcomes.
• Document care management activities and provide feedback on care and quality improvement opportunities.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN according to state contract requirements.
• Behavioral health experience is required.
• Applicants must be able to work remotely from within Missouri.
