Location: Remote Illinois; must reside in Illinois or a border state within a commutable distance of Illinois. Requires up to 25% travel for field-based member interactions and occasional meetings at 's Illinois offices. Working hours are Monday–Friday, 8:00 AM–5:00 PM CST, with flexibility based on business needs.
Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do
- Coordinate care for pediatric members with chronic conditions, complex medical needs, developmental disabilities, behavioral health conditions, and medically fragile conditions.
- Conduct telephonic and face-to-face assessments, develop individualized care plans, and coordinate care management activities.
- Collaborate with families, caregivers, providers, community resources, medical directors, and interdisciplinary care teams.
- Identify and resolve barriers to effective care while monitoring progress toward desired outcomes.
- Coordinate physical, behavioral, developmental, medical, and psychosocial support across the continuum of care.
- Maintain contractual and regulatory compliance and make independent decisions in ambiguous situations.
Requirements
- Associate degree in nursing required.
- At least 3 years of clinical experience and 2 years of care management experience.
- At least 1 year of acute care clinical experience in a hospital, emergency department, home health, rehabilitation, ambulatory care, skilled nursing, behavioral health, case management, disease management, utilization management, or clinical triage setting.
- Knowledge of community health, social service agencies, and community resources.
- Ability to use electronic medical records and other electronic information applications, with strong keyboard and web navigation skills.
- Valid state driver's license, personal vehicle liability insurance, and willingness to travel up to 25% are required.
Nice to have
- Bachelor of Science in Nursing, case management certification, or motivational interviewing certification.
- In-home assessment, care coordination, health promotion, coaching, wellness, or managed care experience.
- Experience with early intervention, individualized education programs, pediatric behavioral health, autism spectrum disorders, ADHD, developmental disabilities, or medically complex children.
- Bilingual ability in Spanish, Arabic, Vietnamese, or another language.
- Knowledge of pediatric developmental milestones and age-appropriate care management interventions.
