Maternal Field Care Manager, Nurse

Location: Remote, but must reside in Illinois or a border state within a commutable distance of Illinois. The role includes up to 25% field-based travel and occasional meetings at hirify.global locations in Illinois. Work hours are generally Monday–Friday, 8:00 AM–5:00 PM CST, with schedule flexibility as needed.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and face-to-face assessments for pregnant and postpartum members with moderate to high acuity needs.
• Create individualized care plans and coordinate physical, behavioral, and psychosocial healthcare services.
• Identify and resolve barriers to effective care while monitoring progress toward desired outcomes.
• Coordinate with providers, medical directors, internal departments, community resources, and interdisciplinary care teams.
• Participate in interdisciplinary care team meetings and support cost-effective use of available resources.
• Maintain compliance with contractual, regulatory, and organizational requirements.

Requirements

• Associate degree in nursing and at least 3 years of nursing experience.
• At least 2 years of care management experience.
• At least 1 year of acute care experience in obstetrics, women's care, labor and delivery, mother-baby, NICU, or clinical triage.
• Knowledge of community health, social service agencies, and community resources.
• Ability to use electronic medical records and other electronic information systems, with strong keyboard and web navigation skills.
• Valid state driver's license, personal vehicle liability insurance, and willingness to travel up to 25% are required.

Nice to have

• Bachelor of Science in Nursing, in-home assessment and care coordination experience, or managed care experience.
• Lactation experience, preferably as a certified lactation consultant.
• Experience in health promotion, coaching, wellness, motivational interviewing, or case management certification.
• Experience with pregnant and postpartum populations with moderate to high acuity.
• Bilingual ability in Spanish, Arabic, Vietnamese, or another language.
