Care Navigator - LPN / LVN (Healthcare)

Location: Remote from anywhere within the state of Missouri; Monday-Friday, 8am-5pm Central. The role may require on-site member visits when appropriate.

Salary: $22.94-$38.79 per hour.

What you will do

• Assess member needs, barriers to care, available resources, and recommended services.
• Develop or contribute to personalized care plans and identify appropriate providers, specialists, and community resources.
• Coordinate care between members, caregivers, healthcare providers, care managers, and community resources.
• Educate members, families, caregivers, and care managers about care options, referrals, procedures, and healthcare benefits.
• Monitor care plan progress and member status, collaborating with providers on revisions or additional evaluations.
• Document member information and care management activities in compliance with state, federal, and payer requirements.

Requirements

• Bachelor’s degree and 2-4 years of related experience, or equivalent applicable experience.
• Graduation from an accredited school of nursing if holding clinical licensure.
• Current Missouri LPN/LVN license is highly preferred.
• Ability to work remotely from within Missouri.
