Location: Ohio, United States; candidates must reside in Ohio and remain within a reasonable commuting distance of the posting locations. This field-based role involves travel to client sites or designated locations, with occasional office attendance for meetings or training. ’s hybrid workforce strategy requires onsite work at an location at least once per week unless the role is designated primarily virtual.
Salary: $30.89–$38.61 per hour in Columbus, Ohio.

What you will do
- Support LTSS care coordination for members across physical health, behavioral health, social services, and long-term support needs.
- Assist with telephonic and face-to-face assessments within the scope of licensure.
- Identify members at risk of complications and collect clinical data for the responsible RN.
- Identify members who may benefit from alternative levels of care or waiver programs.
- Coordinate care for members with chronic illnesses, co-morbidities, and disabilities alongside the RN, member, and healthcare team.
- Apply knowledge of disease processes, terminology, and clinical guidelines to support cost-effective and efficient benefit utilization.
Requirements
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or another non-RN license permitted by applicable state law.
- LPN/LVN candidates must have a Nursing Diploma or Associate degree in Nursing or a related field.
- At least two years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in service coordination or a similar role.
- Must reside in Ohio and be able to work within commuting distance of a posting location.
- Ability to travel to client sites or designated locations and attend office meetings or training as required.
Nice to have
- Case management experience with older adults or individuals with disabilities.
- Bachelor’s degree in Health or Nursing.
