LPN Escalation Nurse (CCM/RPM)

Location: Fully remote; current active compact LPN/LVN license required

Company

Provides chronic care management and remote patient monitoring services for patients with multiple long-term chronic conditions.

What you will do

• Serve as an escalation point for outbound, inbound, and enrollment teams when patient concerns exceed the initial caller’s scope of practice.
• Review patient histories, vital signs, and remote monitoring readings to determine urgency and appropriate next steps.
• Conduct patient outreach for escalated concerns within LPN scope of practice.
• Monitor remote monitoring alerts and out-of-range readings, determining when escalation or routine follow-up is needed.
• Escalate concerns requiring RN or provider assessment and coordinate with ordering providers’ offices.
• Document escalation activity, assessments, outcomes, and follow-up in compliance with clinical requirements.

Requirements

• Current active compact LPN/LVN license
• LPN/LVN degree from an accredited nursing program
• Strong triage, critical-thinking, and prioritization skills.
• Ability to work independently and apply sound clinical judgment within LPN scope of practice.
• Strong communication skills for coordinating with providers’ offices and the RN Escalation team.
• Ability to follow clinical escalation protocols and remain composed when handling urgent or sensitive concerns.
