LTSS Svc Coordinator-Clinician

Location: Field-based role in Nashville, Tennessee, United States; candidates must be within a reasonable commuting distance and travel to client sites or designated locations, with occasional office attendance.

What you will do

• Support the responsible RN in identifying members at risk of complications.
• Collect clinical data and assist with telephonic or face-to-face member assessments.
• Identify members who may benefit from alternative levels of care or waiver programs.
• Coordinate care for members with chronic illnesses, co-morbidities, and disabilities alongside the RN, member, and healthcare team.
• Apply disease-process knowledge, clinical terminology, and clinical guidelines within the scope of licensure.
• Provide collected information to the RN for verification, interpretation, and care-plan development.

Requirements

• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or another non-RN license permitted by applicable state law.
• Nursing Diploma, Associate degree in Nursing, or a related field.
• At least two years of experience working with individuals with chronic illnesses, co-morbidities, or disabilities in a service coordinator or similar role.
• Must be within a reasonable commuting distance of the Nashville posting location unless a legally required accommodation applies.
• Travel to client sites, the worksite, and other locations is required as needed.

Nice to have

• Master’s degree in Health or Nursing.
• State-specific certification required by applicable law or contract.
