TL;DR

LPN Personal Care Coordinator (Chronic Care Management): Supporting patients with chronic conditions through monthly outbound care-management calls, resource coordination, documentation, and health education with an accent on individualized care plans and patient advocacy. Focus on identifying changing care needs, addressing barriers to services, escalating concerns to triage nurses, and maintaining accurate clinical records.

Location: Fully remote; day shift, Monday through Friday. Equipment is provided.

ChartSpan provides chronic care management services for patients with multiple chronic conditions between office visits.

What you will do

Conduct monthly outbound care-management conversations with assigned patients.
Identify patient-specific problems, goals, interventions, barriers to care, and changes requiring care-plan updates.
Connect patients with appropriate healthcare resources and services.
Provide health education and ongoing patient support.
Maintain accurate clinical documentation, including medical history, medications, immunizations, allergies, surgical history, and family history.
Escalate patient concerns to the triage nurse team and meet utilization, call-scheduling, quality, and attendance metrics.

Requirements

Current active compact LPN/LVN license.
LPN/LVN degree from an accredited nursing program.
Fluent Spanish required.
Ability to follow established clinical protocols and procedures.
Flexibility when switching between tasks and willingness to support coworkers.
