Quality Care Member Advocate - Clinical

Location: Must be based in the Dallas–Fort Worth, Texas area; hybrid schedule with 50% work from home and 50% in-person member engagement through home and community visits. Monday–Friday, 8:00 a.m.–5:00 p.m. CST.

Salary: $27.02–$48.55 per hour.

What you will do

• Conduct community outreach, home visits, and community-based assessments to identify care gaps and social determinants of health.
• Help members schedule appointments, understand care plans, access benefits, and navigate healthcare and social service systems.
• Educate members about preventive care, chronic condition management, and available community resources.
• Collaborate with providers and internal Quality, Care Management, and Provider Relations teams using quality data such as HEDIS and CAHPS.
• Document member interactions, referrals, and care gap closures in the appropriate systems.
• Monitor outreach effectiveness, report care gap metrics, support seasonal campaigns, and build relationships with community organizations.

Requirements

• Bachelor’s degree in Social Work, Public Health, Nursing, or a related field, or equivalent experience.
• At least 3 years of experience in care coordination, care management, community or public health education, quality measures, health screening assessments, or preventive care outreach.
• Experience supporting members with physical health conditions and chronic disease management, including diabetes, hypertension, high cholesterol, or stress-related conditions.
• Knowledge of Medicaid, Medicare, HEDIS, and other healthcare quality measures.
• Strong clinical documentation, communication, problem-solving, organization, attention to detail, and time management skills.
• RN state or compact licensure and/or LCSW certification are preferred.
