TL;DR
Quality Care Member Advocate - Clinical (Healthcare Quality and Care Coordination): Improving member health outcomes by identifying and closing care gaps through community outreach, home visits, member education, and care coordination with an accent on preventive care, chronic condition management, and healthcare navigation. Focus on conducting field-based assessments, documenting interventions, collaborating with providers on HEDIS and CAHPS performance, and monitoring care gap closure metrics.
Location: Must be based in the Dallas–Fort Worth, Texas area; hybrid schedule with 50% work from home and 50% in-person member engagement through home and community visits. Monday–Friday, 8:00 a.m.–5:00 p.m. CST.
Salary: $27.02–$48.55 per hour.

What you will do
- Conduct community outreach, home visits, and community-based assessments to identify care gaps and social determinants of health.
- Help members schedule appointments, understand care plans, access benefits, and navigate healthcare and social service systems.
- Educate members about preventive care, chronic condition management, and available community resources.
- Collaborate with providers and internal Quality, Care Management, and Provider Relations teams using quality data such as HEDIS and CAHPS.
- Document member interactions, referrals, and care gap closures in the appropriate systems.
- Monitor outreach effectiveness, report care gap metrics, support seasonal campaigns, and build relationships with community organizations.
Requirements
- Bachelor’s degree in Social Work, Public Health, Nursing, or a related field, or equivalent experience.
- At least 3 years of experience in care coordination, care management, community or public health education, quality measures, health screening assessments, or preventive care outreach.
- Experience supporting members with physical health conditions and chronic disease management, including diabetes, hypertension, high cholesterol, or stress-related conditions.
- Knowledge of Medicaid, Medicare, HEDIS, and other healthcare quality measures.
- Strong clinical documentation, communication, problem-solving, organization, attention to detail, and time management skills.
- RN state or compact licensure and/or LCSW certification are preferred.

- Bilingual English/Spanish skills.
