Community Health Worker

Location: Remote Oklahoma, with required in-person visits to member locations; occasional travel to hirify.global offices for training or meetings may be required.

Salary: $41,900–$56,600 per year

What you will do

• Conduct in-person and telephone non-clinical assessments of member needs, preferences, home environments, and socioeconomic barriers.
• Coordinate physical and behavioral health appointments, social services, community resources, and referrals.
• Collaborate with care managers, Wellness and Information Navigators, and multidisciplinary care teams.
• Advocate for members with providers, schools, community organizations, and other support services, including accompanying members to appointments.
• Support care-plan, medication, and treatment adherence through culturally appropriate education and coaching.
• Attend community events, build provider and community-resource relationships, and identify gaps in local services.

Requirements

• High school diploma or equivalent.
• At least two years of experience with community resources, health agencies, or social services.
• Must reside in Oklahoma, work full time, and travel to member locations.
• Intermediate proficiency in Microsoft Office.
• Strong written communication, member advocacy, and respect for cultural and demographic diversity.
• Dedicated workspace and internet service with at least 25 Mbps download and 10 Mbps upload are required.

Nice to have

• Community Health Worker training or certification, or willingness to complete it within one year.
• Bachelor’s degree in social work or a related field.
• Familiarity with state Medicaid guidelines and experience with Medicaid enrollees.
• Fluency in Spanish, French Creole, or Vietnamese.
