Manager, Care Management

Location: Remote from Illinois; requires 50–75% field-based travel and occasional onsite meetings at hirify.global's Illinois locations. Monday–Friday, 8:00 AM–5:00 PM CST, with schedule flexibility based on business needs.

Salary: $86,300–$118,700 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead teams of nurses, social workers, behavioral health professionals, and care management support associates, with up to 15 direct reports.
• Oversee member needs assessments and care planning for Medicaid programs, including MLTSS or LTSS.
• Develop care management models using community health workers, peer support specialists, housing advocates, and other extenders.
• Build partnerships with community organizations, aging services, housing providers, public agencies, members, and families.
• Monitor program performance, analyze data trends, identify service gaps, and improve cost, quality, and member satisfaction.
• Maintain policies, procedures, compliance, training, and operational coordination across the care management department.

Requirements

• Must reside in Illinois.
• Associate's degree or higher.
• Active Registered Nurse license or Social Work license.
• At least 5 years of professional experience and at least 2 years of management or supervisory experience.
• Knowledge of Medicaid, Medicare, long-term care guidelines, benefits, and policies.
• Experience with data analysis, operational leadership, cross-functional collaboration, communication, problem solving, and Microsoft Office applications.
• Valid state driver's license and personal vehicle liability insurance are required.
• Work requires a dedicated home workspace protecting member PHI and HIPAA information, plus internet speeds of at least 25 Mbps download and 10 Mbps upload.

Nice to have

• Bachelor's or advanced degree in nursing or a health-related business field.
• Managed care experience and five or more years of management or supervisory experience.
• Experience with community health workers, peer support specialists, housing programs, or Illinois Medicaid systems.
