Field Care Coordinator

Location: Clearwater, Florida; serving Pasco and Pinellas counties with 75–90% local travel to members' homes, assisted living facilities, long-term care facilities, and other care settings.

Salary: $60,000–$65,000 per year based on experience and qualifications. The broader posted pay range is $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs and barriers and create individualized care plans.
• Plan and implement interventions that support optimal wellness and the least restrictive care setting.
• Coordinate services with providers, payer sources, interdisciplinary teams, families, and community resources.
• Monitor and evaluate case management plans against members' personal goals and health outcomes.
• Guide members and families toward appropriate care and social services.

Requirements

• One of the following: a bachelor's degree in Health Services, Social Work, Psychology, or a related field with at least 2 years of relevant experience; a Florida-licensed RN with at least 2 years of relevant experience; or a Florida-licensed LPN with at least 4 years of relevant experience.
• At least 2 years of case management experience and prior experience with Medicare and Medicaid recipients.
• Monday–Friday schedule, 8:30 a.m.–5:00 p.m.; 40 scheduled hours per week.
• Valid state driver's license and proof of personal vehicle liability insurance are required.
• AHCA fingerprinting and tuberculosis screening are required for this patient-facing role.

Nice to have

• Nursing home diversion or long-term care case management experience.
• Bilingual English/Spanish communication skills.
• Experience with electronic case-note documentation and multiple computer systems.
• Experience with geriatric populations, health promotion, coaching, wellness, and community health resources.
• 701B certification or medication application (RFA) experience.
