Clinical Care Navigator, Outbound (Saturday-Wednesday 9am-5pm ET)

Location: Remote; required Saturday-Wednesday coverage from 9am-5pm ET or the equivalent in the respective time zone.

Salary: $33.85-$38.93 per hour, plus stock options and benefits.

What you will do

• Provide outbound clinical support, psychoeducation, referrals, and resources, including some inbound crisis line coverage.
• Engage members seeking help with mental health, substance use, or family and relationship concerns using active listening, motivational interviewing, and solution-focused approaches.
• Assess risk, provide crisis support and safety planning, and follow up with high-risk members.
• Develop action plans and coordinate care with therapists, coaches, physicians, external resources, and Provider Operations.
• Complete professional documentation and communicate with members and providers by email.
• Participate in case consultations, training, onboarding, mentoring, and multidisciplinary problem-solving.

Requirements

• Active license as an LCSW, LMFT, LPC, LICSW, or licensed psychologist.
• At least 2 years of direct clinical care experience.
• Expertise in crisis assessment and safety planning.
• Knowledge of crisis support, CBT, motivational interviewing, mindfulness, and solution-focused clinical interventions.
• Excellent listening, written, and verbal communication skills, with the ability to multitask and context-switch.
• Comfort using multiple computer platforms and learning new systems.
