Care Manager, Telephonic Behavioral Health

Location: Remote Illinois; occasional travel to hirify.global offices for training or meetings may be required. Working hours are between 8:00 a.m. and 7:00 p.m. Central Standard Time, with core hours from 8:00 a.m. to 5:00 p.m.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members' physical, psychological, emotional, environmental, and psychosocial needs by telephone.
• Support members experiencing mental illness and complex medical conditions through ongoing care management.
• Develop and coordinate interventions, referrals, education, resources, and community services.
• Conduct proactive outreach and engage eligible members in appropriate care management programs.
• Collaborate with interdisciplinary teams including social services, pharmacists, psychiatrists, utilization management, and community health educators.
• Use motivational interviewing and solution-oriented communication while documenting care and meeting performance standards.

Requirements

• Bachelor's degree.
• At least 3 years of experience as a Licensed Clinical Social Worker or Licensed Clinical Professional Counselor.
• Illinois licensure in the relevant field, with no disciplinary action, or the ability to obtain Illinois licensure.
• At least 1 year of managed care experience.
• Experience and commitment to improving consumer healthcare experiences.
• Dedicated, interruption-free home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds.

Nice to have

• Master's degree in behavioral health, social work, psychology, or a related healthcare discipline.
• Bilingual ability in English and Spanish, Polish, or Creole; language proficiency testing is required for member communication in a non-English language.
• Certified Case Manager certification.
• Experience with case management, discharge planning, adult acute-care education, Medicare and Medicaid recipients, electronic case documentation, health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and community resources.
