Transitions of Care Behavioral Health Social Work Case Manager

Location: Fully remote within the United States; must be located in a state where Devoted operates. Schedule: Monday–Friday, 8:30 a.m.–5:00 p.m. PST or MST.

Salary: $55,000–$79,000 per year.

What you will do

• Provide short-term transitions-of-care support to members connecting with community-based behavioral health services after hospitalization or when new needs emerge.
• Conduct comprehensive psychosocial assessments with members and caregivers by phone and video.
• Deliver brief therapeutic interventions, address family and support issues, assess social determinants of health, and complete safety assessments.
• Collaborate with clinicians, nurses, and care coordinators to deliver cohesive, patient-centered care for high-needs members.
• Identify and escalate complex cases to the Behavioral Health Medical Director for consultation.
• Document clinical interactions in the electronic health record and participate in huddles, panel reviews, and interdisciplinary rounds.

Requirements

• Master’s degree in Social Work.
• Active clinical license in any U.S. state, such as LCSW, LMSW, or a comparable state clinical license; willingness to obtain additional state licenses.
• At least 5 years of experience in behavioral health or care management.
• Experience conducting comprehensive psychosocial assessments by video and phone.
• Ability to independently manage a fully remote schedule during the required PST or MST hours.

Nice to have

• Bilingual English and Spanish skills.
• Creole or Vietnamese language skills.
• Experience in geriatric behavioral health or the health insurance industry, especially Medicare Advantage.
