Care Manager (Behavioral Health)

Location: Remote in Illinois; applicants must reside in one of the specified Illinois counties. The role is work from home with 10% travel for quarterly team meetings and may include telephonic, digital, home, and other site visits.

Salary: $56,200–$101,000 per year

What you will do

• Develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs.
• Create and maintain personalized care plans for members with high-acuity needs, including mental health and substance use disorder support.
• Assess member needs through phone, digital outreach, home visits, and other site visits, then connect members with providers, specialists, and community resources.
• Coordinate care among members, families, caregivers, healthcare providers, and community organizations to ensure timely access to services.
• Monitor treatment outcomes, symptoms, side effects, complications, and changes in member status, updating care plans as needed.
• Document care management activities, educate members and families, and provide feedback on care quality and delivery improvements.

Requirements

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• For the Illinois YouthCare plan, hold a graduate degree and independent Illinois behavioral health licensure, such as LCSW, LCPC, or PsyD, or hold a bachelor's degree and Illinois RN licensure.
• Must reside in Illinois in one of the specified eligible counties.
• Knowledge of the Child Welfare System or DCFS is helpful.
