Field Care Manager Nurse 2 - BSN (Behavioral Health)

Location: Remote position for candidates who must reside in South Carolina and hold an active South Carolina nursing license; travel is required 75%–90% of the time.

Salary: $71,100–$97,800 per year, plus eligibility for a performance-based bonus incentive.

What you will do

• Assess and evaluate members’ physical, environmental, psychosocial, and behavioral health needs.
• Guide members and families toward appropriate healthcare, community health, and social service resources.
• Identify and resolve barriers that prevent effective care and support progress toward desired outcomes.
• Create and maintain member care plans while continuously monitoring care through assessments and evaluations.
• Travel frequently to conduct face-to-face meetings and collaborate with members, families, providers, and staff.

Requirements

• Bachelor’s degree in nursing (BSN) and an active South Carolina nursing license without disciplinary action.
• Must reside in South Carolina.
• At least 2 years of case or care management experience.
• At least 2 years of experience working with behavioral health populations.
• Knowledge of community health, social service agencies, and community resources.
• Ability to use electronic information systems and electronic medical records.

Nice to have

• At least 3 years of in-home assessment and care coordination experience.
• Experience with health promotion, coaching, wellness, or managed care.
• Spanish bilingual ability.
• Case Management or Motivational Interviewing certification or related knowledge.
