Visiting Nurses Association - Physical Therapy Assistant (Physical Therapy)

Location: Manchester, New Hampshire, United States; in-home care in Manchester and surrounding communities. Work shift: 40 hours per week, Monday–Friday, 8:00 AM–4:30 PM.

What you will do

• Administer physical therapy programs under the direction of the supervising or referring Physical Therapist.
• Support rehabilitation progress through regular consultation with the supervising or referring Physical Therapist.
• Document patient care according to VNA standards and processes using a laptop computer.
• Develop and implement discharge plans, including home exercise programs, equipment recommendations, patient and family education, and community referrals.
• Coordinate care with physicians, nursing staff, and interdisciplinary team members.
• Follow safety, ethical, confidentiality, policy, protocol, and infection-control standards.

Requirements

• Graduate of an accredited Physical Therapy Assistant program with an associate degree.
• Licensed as a Physical Therapist Assistant by the New Hampshire Office of Licenses Allied Health Professionals.
• Current BLS and CPR certification.
• Minimum one year of experience in a healthcare setting performing similar work is preferred.
• Basic typing skills and ability to document on a laptop computer.
