What you will do

• Assess patients’ physical functioning, including relevant OASIS data elements, and determine their need for physical therapy services.
• Develop plans of care with physicians, patients, and caregivers to support maximum rehabilitation potential.
• Deliver prescribed in-home physical therapy treatments and supervise Physical Therapy Assistants.
• Establish and instruct patients in home exercise programs and document care accurately.
• Monitor changes in patient functioning, communicate updates to physicians and care team members, and maintain confidential records.

Requirements

• Bachelor’s degree from an accredited physical therapy program recognized by the Council on Medical Education of the American Medical Association.
• Current physical therapist license or registration in the State of Texas is required.
• At least one year of current physical therapist experience and at least one year of experience under the supervision of a Registered Physical Therapist.
• Valid state driver’s license and personal vehicle liability insurance with limits of at least 100,000/300,000/100,000 are required.
• Ability to comply with applicable state, federal, local, and agency policies and regulations, including HIPAA.
