Physical Therapist Assistant - Home Health

Location: Boise, Idaho, United States; home visits throughout the assigned service area

What you will do

• Provide physical therapy treatments in patients' homes under the direction and supervision of a licensed Physical Therapist.
• Follow individualized treatment plans and assist with exercises, stretching, strengthening, balance, mobility, and functional activities.
• Monitor patient responses, recognize changes in condition, and report progress or concerns to the supervising Physical Therapist.
• Educate patients and caregivers on exercises, mobility techniques, safety, and assistive devices.
• Maintain accurate clinical documentation and communicate with patients, families, physicians, and the healthcare team.
• Manage a daily schedule while traveling between patient homes and following home health policies.

Requirements

• Graduate of an accredited Physical Therapist Assistant program.
• Current Physical Therapist Assistant license in Idaho required.
• Current CPR/BLS certification, as required.
• Strong communication, interpersonal, organizational, and documentation skills.
• Ability to work independently in a home health environment and travel throughout the assigned service area.
• Reliable transportation and a valid driver's license.
