Visiting Nurse Association- Physical Therapist- Home Care- Full Time- Days

Location: Manchester, New Hampshire, with travel throughout southern New Hampshire for home health care

Compensation: $15,000 bonus eligible

What you will do

• Plan, develop, implement, and update individualized physical therapy care plans.
• Provide age-appropriate physical therapy services to patients of all ages and their families in the home.
• Re-evaluate patient needs and confirm that care plans remain appropriate and aligned with physician orders.
• Communicate care information with patients, physicians, peers, managers, departments, and external agencies.
• Apply problem-solving and quality-improvement methods to patient care and organizational processes.
• Orient and supervise physical therapist assistants in accordance with New Hampshire guidelines and VNA policy.

Requirements

• Degree from an American Physical Therapy Association-accredited program.
• New Hampshire Physical Therapist license required.
• BLS and CPR certification required.
• At least one year of experience in a healthcare setting performing similar work is preferred.
• Valid driver's license, automobile insurance meeting minimum coverage requirements, and ability to travel throughout southern New Hampshire are required.
• Basic typing skills and ability to document care using a laptop computer.
