Care Manager - Foster Care (BH)

Location: Remote from anywhere within the state of Missouri; Monday-Friday, 8am-5pm.

Salary: $56,200-$101,000 per year.

What you will do

• Assess members’ mental, behavioral health, substance use disorder, and social determinant needs by phone, digitally, in the home, or at other sites.
• Develop and maintain personalized care plans for members with high-acuity needs.
• Coordinate timely access to providers, specialists, community resources, and other required services.
• Monitor treatment symptoms, side effects, complications, member status, and care outcomes.
• Educate members and families about treatment options, referrals, provider instructions, procedures, and healthcare benefits.
• Document care management activities and provide feedback on quality and cost-effective care improvements.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2-4 years of related experience.
• Licensed Master’s Behavioral Health Professional credential, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN licensure based on state contract requirements.
• Behavioral health experience is required.
• Applicants must be able to work remotely from within Missouri.
