TL;DR
Senior LTSS Service Care Manager RN - Pediatrics (Long-Term Care): Assessing and coordinating medical and support services for complex pediatric long-term care members with an accent on personalized care plans, provider collaboration, and family education. Focus on monitoring high-acuity conditions, coordinating community resources and service authorizations, and improving quality of care across the continuum.

Location: Hybrid role based in Lubbock, Texas, with in-home visits in the Abilene/Midland area.

Salary: $36.21–$65.09 per hour

What you will do

  • Assess complex and high-acuity pediatric long-term care members and develop personalized care and service plans.
  • Coordinate providers, specialists, caregivers, facilities, agencies, and community resources to meet member needs.
  • Monitor member status, complications, care gaps, service-plan progress, waiver eligibility, and potential transitions to higher levels of care.
  • Educate members and families on disease processes, care options, referrals, provider instructions, benefits, and available community services.
  • Coordinate service authorizations for resources such as transportation, housing, meals, foster care, and activities of daily living.
  • Document care management activities, support regulatory compliance, and contribute to quality and cost-effective service delivery.

Requirements

  • Graduate from an accredited school of nursing or hold a bachelor's degree with 4–6 years of related experience.
  • RN state licensure or compact state licensure required; alternatively, current state nurse licensure as a nurse practitioner.
  • Resource Utilization Group (RUG) certification must be obtained within 90 days of hire.
  • Ability to work in the Lubbock area and complete in-home or other site visits in the Abilene/Midland area.
  • Strong skills in care coordination, clinical assessment, documentation, member advocacy, and collaboration with healthcare providers.

Nice to have

  • Bachelor's degree in Nursing.
  • Experience precepting, coaching, or onboarding clinical new hires.
